You only need to fill out this form once, even if you order multiple products. Regardless of when you activate the warranty, it will expire 5 years after the day you physically receive the product.
Wasauna ships all of our showers and bathtubs with R&L Carriers (http://www.rlcarriers.com). Please inspect your box to make sure that the product is in good shape when it arrives. If you notice any issues, please note them on the bill of lading before the delivery team leaves.
